Insurance & Self-Pay
We accept most major private insurance plans. However, due to frequent changes in coverage and narrow network plans, we recommend confirming with your insurance provider that either Be Well Primary Care LLC or Holly Lukenbill, FNP-BC is in-network prior to your visit.
We also offer transparent self-pay options for patients without insurance — because quality care should always be accessible.
Insurance Coverage Details
We are currently in-network with Aetna, Medicaid, and Anthem, and credentialing with additional insurers is underway. As new networks are approved, we’ll update this list. Please note that even if your insurer is listed, it’s important to verify coverage—each company offers multiple network plans that may vary in participation.
